Support Coordination Level 1: Support Connection
For participants with basic needs –

Support Coordination Level 2: Support Coordination
For participants with complex needs –
- Help manage and implement your NDIS plan.
- Helping you develop the skills you need to manage your own plan.
- Offer assistance you need to connect with your providers and understand service agreements.
Support Coordination Level 3: Specialist Support Coordination
For participants with high-intensity needs –
- Help with crisis management.
- Coordination of support.
- Help coordinate healthcare services.
